Conduct in-home and virtual health evaluations for health plan members, reviewing their health history and medications, performing physical evaluations and applicable diagnostic and preventive services, and helping connect people to appropriate care.
What you'll do
- Conduct in-home evaluations and virtual visits as needed, including medical and medication history reviews and physical evaluations.
- Provide indicated Diagnostic & Preventive Services, such as spirometry, DEE, and bone density screening.
- Travel locally and within licensed states as needed, and support referrals and care coordination.
