Conduct in-home and virtual health evaluations for health plan members, reviewing medical history and medications, performing physical evaluations and indicated diagnostic and preventive services, and helping connect members with appropriate care.
What you'll do
- Conduct in-home evaluations and virtual visits as needed, including medical history and medication reviews and physical evaluations.
- Provide indicated Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screening.
- Travel locally and within licensed states as needed, and help connect members with appropriate care.
