Conduct in-home and virtual health evaluations for health plan members, reviewing medical history and medications, performing physical evaluations, and providing indicated diagnostic and preventive services.
What you'll do
- Conduct approximately 45-minute in-home evaluations and virtual visits as needed.
- Provide indicated Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screening.
- Travel locally and within licensed states as needed, and connect members with appropriate care.
