Conduct in-home health evaluations for health plan members across a defined coverage area, helping identify care needs and connect people with appropriate care.
What you'll do
- Perform approximately 45-minute home visits, including medication and medical history review and physical evaluation; provide ordered diagnostic and preventive services.
- Conduct virtual visits as needed and travel locally and within licensed states, potentially including overnight stays.
- Use an iPad and clinical workflow to document evaluations; prescribing, ordering lab tests, and changing treatment regimens are not required.
