Provides in-home health evaluations and, as needed, virtual visits for health plan members across Minnesota and other licensed coverage areas.

- Travel locally and across licensed states, with possible overnight stays, car rentals, or flights; conduct virtual visits as needed.
- Connect members with appropriate care and help address gaps in care.
