Conduct in-home and occasional virtual health evaluations for health plan members, reviewing medical history and medications, performing physical evaluations and ordered diagnostic services, and connecting people with appropriate care.
What you'll do
- Conduct in-home evaluations and virtual visits as needed, including medical history and medication reviews and physical evaluations.
- Provide diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Travel locally and across licensed states to serve members in their homes.
