- Travel across assigned coverage areas, potentially up to 100% of the time, to conduct in-home evaluations.
- Review members’ medications and medical history and perform physical evaluations and indicated diagnostic or preventive services.
- Conduct virtual visits as needed and help connect members with appropriate care.
