About the job
Provides field-based case management for members and families, coordinating assessments, care plans, services, and support to improve health outcomes.
What you'll do
- Assess member needs and risks, recommend services, and coordinate and monitor care plans.
- Collaborate with care teams and providers to address barriers, improve care quality, and engage members in health decisions.
- Document care and follow case management processes in accordance with regulations and company policies.
What they ask for
- At least 6 months of experience in a listed area relevant to Kentucky SKY populations, such as foster care, behavioral health services, trauma-informed care, or crisis intervention.
- At least 2 years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling.
