About the job
Provides community-based nursing care by assessing patients, administering treatments and medications, documenting care, and coordinating access to health resources.
What you'll do
- Administer medications and treatments; assess patient conditions, monitor vital signs and pain, and document responses.
- Conduct medication reviews and mental health screenings, and maintain accurate patient records in electronic health records.
- Connect patients with healthcare and community resources, communicate with care teams, and maintain a clean, stocked clinic.
