Provides psychosocial assessments, advocacy, and care coordination for high-risk patients and their caregivers as part of an interdisciplinary care team.
What you'll do
- Assess and support patients through check-ins, visits, care planning, crisis response, and coordination of care.
- Connect patients with community resources, navigate insurance benefits, and counsel patients and families on supportive care and disease-related issues.
- Collaborate with care teams and Community Health Workers, and document interventions.
