Provides psychosocial assessments, advocacy, and care coordination for high-risk patients and their caregivers to support health goals and help prevent hospitalizations.
What you'll do
- Conduct patient assessments, check-ins, visits, and ongoing care coordination, including identifying goals and addressing barriers to care.
- Connect patients with community resources, help them navigate insurance benefits, and provide education and counseling on supportive care.
- Collaborate with interdisciplinary care teams and Community Health Workers, document interventions, and respond to urgent or complex needs.
