About the job
Provides psychosocial assessment, advocacy, and care coordination for high-risk patients and their caregivers as part of an interdisciplinary care team.
What you'll do
- Assess and support patients through check-ins, visits, goal-setting, referrals, and longitudinal or episodic care coordination.
- Educate and counsel patients and families on supportive care, advance care planning, and life-stage or disease-specific concerns.
- Collaborate with care teams and Community Health Workers, respond to crises, and document interventions.
