About the job
Delivers virtual primary and acute care to patients age 18 months and older, including assessment, diagnosis, treatment, counseling, care coordination, and disposition planning.
What you'll do
- Provide evidence-based care, health counseling, medication management, and disposition planning, including contraceptive and STI prevention counseling.
- Document care in the EHR and educate patients on health maintenance.
- Coordinate care with the patient-centered medical home and other healthcare professionals.

- Current license in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV; must be willing and able to obtain additional licensure as needed.
