About the job
Provides value-based outpatient primary care to adults on Medicare, coordinating treatment with physicians and the care team to support patients’ health and prevent hospital visits.
What you'll do
- Provide routine and acute care, annual wellness visits, and health risk assessments.
- Coordinate care with other providers and lead population health activities, including patient outreach and phone triage.
