About the job
Provides value-based primary care to Medicare adults through independent and collaborative outpatient care, care coordination, and population health activities.
What you'll do
- Conduct routine and acute office visits, annual wellness visits, and health risk assessments.
- Coordinate care with providers and specialists, support population health initiatives, and assist with phone triage and outreach.
- Educate patients about health conditions, care plans, and treatments; conduct home visits as needed.
