About the job
Provides value-based primary care to Medicare adults through independent and collaborative patient visits, care coordination, and population health activities.
What you'll do
- Provide routine and acute primary care, annual wellness visits, and health risk assessments.
- Coordinate care with providers and community resources; support phone triage and patient outreach.
- Educate patients on conditions and care plans, lead population health activities, and conduct home visits as needed.
