Provides value-based primary care to Medicare adults through outpatient visits, longitudinal patient relationships, care coordination, and population health work.
What you'll do
- Conduct routine and acute office visits, annual wellness visits, and health risk assessments.
- Coordinate care with providers and specialists, support phone triage and outreach, and educate patients about care plans and treatments.
