About the job
Provides value-based primary care to Medicare adults through independent and collaborative outpatient care, care coordination, and population health activities.
What you'll do
- Provide routine and acute primary care, annual wellness visits, and health risk assessments.
- Coordinate care with other providers and support population health, phone triage, and patient outreach.
- Educate patients about their health conditions, care plans, and treatments; conduct home visits as needed.
