Provides value-based primary care to Medicare patients, including independent and collaborative visits, care coordination, and population health activities in an outpatient team setting.
What you'll do
- Conduct routine and acute office visits, annual wellness visits, and health risk assessments.
- Coordinate care with providers and specialists, support population health efforts, and assist with triage and outreach.
- Educate patients about conditions and care plans; conduct home visits as needed.
