Provides value-based primary care to Medicare adults in an outpatient setting, coordinating with a collaborative care team to support ongoing health and prevent hospitalizations.
What you'll do
- Conduct routine and acute office visits, annual wellness visits, and health risk assessments.
- Coordinate care with providers, specialists, testing facilities, and agencies; support population health activities.
- Educate patients, assist with phone triage and outreach, and conduct home visits as needed.

$126,500 - $148,500 USD base salary; $87,035.00 - $187,460.00 USD base salary
