Provides value-based primary care to Medicare adults through independent and collaborative outpatient care, care coordination, and population health activities.
What you'll do
- Provide primary care, including routine and acute visits, annual wellness visits, and health risk assessments.
- Coordinate care and support population health through collaboration with providers and the care team.
- Educate patients, assist with triage and outreach, and conduct home visits as needed.
