About the job
Provides value-based primary care to Medicare patients through independent and collaborative visits, care coordination, and population health activities.
What you'll do
- Conduct routine and acute office visits, annual wellness visits, and health risk assessments.
- Coordinate care with other providers and support population health, including triage, outreach, and patient education.
- Provide primary care in the clinic and conduct home visits as needed.

$126,500 - $148,500 USD (base salary); $78,331.00 - $168,714.00 USD (base salary)
