Provides value-based outpatient primary care to Medicare adults, coordinating longitudinal care with physicians and the broader care team.
What you'll do
- Conduct routine and acute office visits, annual wellness visits, and health risk assessments.
- Coordinate care with providers and agencies; support population health, phone triage, and patient outreach.
- Educate patients about health conditions, care plans, and treatments; conduct home visits as needed.
