Provides value-based primary care to Medicare adults through independent and collaborative patient care, care coordination, and population health work in an outpatient clinic.
What you'll do
- Provide primary care, including routine and acute visits, annual wellness visits, and health risk assessments.
- Coordinate care, educate patients, and support population health through outreach, triage, and evidence-based screenings.
- Work with the care team and conduct home visits as needed.
