Completes a 12-month adult primary care nurse practitioner fellowship focused on developing clinical skills and caring for older adults in a value-based care model.
What you'll do
- Build a patient panel and provide primary care to older adults with support from preceptors and an interprofessional team.
- Participate in didactic learning designed to build on graduate training.
- Work at Oak Street Health for at least one year after completing the fellowship.
What they ask for
- Family, adult, or adult-gerontology primary care NP graduate (or expected graduate) between May 2025 and December 2026; must not yet have worked as an NP.
- At least one year of adult RN experience within the last five years and a minimum 3.0 NP-program GPA.
- Passed or be in the process of passing NP board examinations by 12/7/26; hold or obtain an active, non-probationary state NP license by 12/21/26.
- Reside in or be willing to relocate to the fellowship location for at least two years.

$110,000 USD (annual salary); $95,738.00 - $206,206.00 USD (base salary)
