Completes a 12-month paid adult primary care nurse practitioner fellowship, building clinical skills and a patient panel while serving older adults in Oak Street Health’s value-based care model.
What you'll do
- Build a patient panel and provide adult primary care to older adults.
- Learn through preceptors and didactic training.
- Collaborate with an interprofessional care team.
