Complete a 12-month adult primary care nurse practitioner fellowship, developing clinical skills and a patient panel while caring for older adults in a value-based care model.
What you'll do
- Build a patient panel and learn through clinical preceptors and didactic education.
- Collaborate with an interprofessional care team to provide primary care for older adults.
- Work at Oak Street Health for at least one year after completing the fellowship.

- Passed or in process of passing NP board examinations by 6/6/2026, and hold or obtain an active, non-probationary state NP license by 8/3/2026.
- Currently reside in or be willing to relocate to the program location for at least two years; Spanish is required.
