Provides value-based primary care to older adult Medicare patients at a neighborhood medical center, with support from a multidisciplinary care team.
What you'll do
- Assess, diagnose, and oversee primary care for patients; prescribe and administer medications.
- Maintain patient electronic medical records and collaborate to meet health quality goals.
- Work with Practice Managers to direct and manage the center care team.
