Provides value-based primary care to Medicare patients at a neighborhood clinic, focusing on assessment, treatment, preventive care, and health outcomes.
What you'll do
- Assess and diagnose patients; oversee and deliver primary care, including prescribing and administering medication.
- Maintain patient records in canopy and greenway and collaborate with care teams and leadership on quality goals.
- Work with Practice Managers to direct and manage the center care team.
