About the job
Provides clinic-based nursing care in a value-based care model, including patient assessment, chronic condition management, education, and coordination with the care team.
What you'll do
- Triage patients and provide protocol-driven chronic care, nursing procedures, medication reviews, and patient education.
- Conduct onsite testing, follow up on abnormal results, and identify gaps in preventive screenings.
- Coordinate care plans and hospital-to-clinic transitions, collaborate with the interdisciplinary team, and delegate tasks as needed.
