Provides clinic-based nursing care focused on chronic condition management, preventive care, patient education, and coordination to improve outcomes and avoid unnecessary hospitalizations.
What you'll do
- Assess and triage patients, deliver nursing procedures, and manage chronic conditions using established protocols.
- Educate patients and caregivers, conduct medication reviews and onsite testing, and respond to abnormal results.
- Coordinate care with providers and the interdisciplinary team, including hospital-to-clinic transitions and delegation to support staff.
