Coordinates patient care at Oak Street Health by providing nursing assessment, triage, education, treatment, and support for care plans and patient outcomes.
What you'll do
- Provide nursing care, including clinical triage, symptom management, medication reviews, point-of-care testing, procedures, and specimen collection.
- Educate patients, coordinate and monitor care plans, and support preventive care and chronic-condition management.
- Collaborate with and delegate to care team members, participate in team meetings, and deliver responsive, respectful patient service.
