Provides clinical nursing care in a value-based clinic, including patient assessment, chronic condition management, education, procedures, and care coordination to improve outcomes and prevent avoidable hospitalizations.
What you'll do
- Triage and assess patients; manage chronic conditions and perform nursing procedures, testing, medication administration, and medication reviews.
- Educate patients and caregivers on chronic conditions and preventive care; identify gaps in screenings and respond to abnormal lab results.
- Coordinate care plans and hospital-to-clinic transitions with providers and the interdisciplinary team; support clinic operations and delegate tasks.
