Leads care management and coordination for complex patients, partnering with primary care providers and interdisciplinary teams to support continuity of care and prevent avoidable admissions.
What you'll do
- Coordinate assigned patients’ care, transitions after discharge, and follow-through across care teams.
- Conduct nursing assessments, medication reconciliation, and patient and family education; document care in the electronic health record.
- Lead in-person interdisciplinary care planning and track admission, readmission, and transitions-of-care metrics.
