What you'll do
- Manage a complex patient caseload and coordinate care across providers, caregivers, and interdisciplinary teams.
- Manage transitions of care, including clinical assessments, medication reconciliation, follow-up, and documentation.
- Conduct nursing assessments, educate patients and families, and lead in-person interdisciplinary care planning meetings.
