Leads care coordination and case management for complex patients, partnering with primary care providers and interdisciplinary teams to support continuity of care and prevent avoidable admissions.
What you'll do
- Manage an assigned caseload and coordinate care, including transitions after hospital discharge and follow-up on patient needs.
- Lead interdisciplinary care planning meetings and collaborate with providers, families, and other care team members.
- Conduct nursing assessments, educate patients and families, and document clinical visits and assessments in the electronic health record.

Pay
$60,522.00 - $129,615.00 USD base salary
