What you'll do
- Manage an assigned caseload and coordinate care, including transitions after hospital discharge and follow-up on identified patient needs.
- Lead interdisciplinary care planning and collaborate with providers, families, and care team members on patient goals and care plans.
- Conduct nursing assessments, educate patients and families, and document clinical assessments and visits in the electronic health record.
