About the job
Provides coordinated primary care nursing, including patient assessment, triage, treatment, education, and care-plan collaboration.
What you'll do
- Provide nursing care, triage, symptom management, medication reviews, and patient education.
- Coordinate and monitor care plans with patients, caregivers, and interdisciplinary clinical teams.
- Perform point-of-care testing, procedures, specimen collection, and other assigned duties.
