Supports members enrolled in a virtual hypertension clinic by assessing cardiovascular health, coaching lifestyle changes, coordinating care, and guiding members through their program journey.
What you'll do
- Assess members’ medical history, medications, adherence, and lifestyle; provide blood pressure monitors and coach accurate self-monitoring.
- Deliver lifestyle and nutrition counseling, using motivational interviewing to help members set and sustain health goals.
- Coordinate follow-ups and multidisciplinary care, document member interactions, and connect members with appropriate resources.
