What you'll do
- Track home health, DME, lab, imaging, prescription, and referral workflows through completion.
- Identify and help close preventive care gaps, educate members, and coordinate screenings and testing.
- Support members through calls and digital messaging, explain care instructions, and coordinate with clinicians and care partners.
