About the job
Provides virtual social work case management for people with complex needs and life-limiting illness, including psychosocial support, care planning, and end-of-life counseling.
What you'll do
- Conduct psychosocial assessments, create individualized care plans, and provide emotional and caregiver support.
- Lead goals-of-care and advance-care-planning conversations, support hospice transitions, and coordinate referrals and community resources.
- Collaborate with patients, caregivers, and interdisciplinary teams; document clinical interactions in the electronic health record.
