Leads and delivers team-based primary care while supervising Advanced Practice Providers and helping build quality measures, clinical processes, and AI-enabled virtual care.
What you'll do
- Manage complex and chronic-care patients as part of an interdisciplinary team; lead weekly rounds and readmission conferences.
- Supervise, teach, and provide quality oversight to Advanced Practice Providers, including collaborative practice agreements across multiple states.
- Improve primary care delivery by developing quality measures, overseeing quality, and advancing AI- and digitally enabled processes.
