Provides virtual primary care, managing chronic and acute conditions, preventive care, medications, and coordination of follow-up services through telehealth.
What you'll do
- Conduct virtual primary care visits, assessments, preventive screenings, and acute care interventions.
- Manage chronic conditions, diagnostics, prescriptions, and individualized care plans.
- Educate and coach patients, coordinate referrals and in-person services, and collaborate with the care team.

- Active Nurse Practitioner (APRN) or Physician Assistant (PA) license in good standing, with ability to obtain licensure in multiple states within 90 days of hire.
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.

Pay
$120K-155K base range (USD); $10K-$20K performance based bonus (USD); $130K-170K total comp range (USD)
