Provide preventative, proactive primary care to members—primarily through telehealth—by conducting assessments, supporting chronic conditions, and coordinating member-centered care.
What you'll do
- Conduct video visits and limited occasional in-home visits; perform comprehensive assessments and provide evidence-based preventative and chronic condition care.
- Identify care gaps, coordinate with care teams, and contribute to individualized whole-person care plans.
- Use integrated technology and AI-enabled workflows to reduce administrative work and increase member interaction time.
