Provides primarily telehealth preventative and primary care assessments, chronic condition support, and coordinated care for older adults and other members.
What you'll do
- Conduct video visits and occasional local in-home visits, performing comprehensive assessments and evidence-based preventive care.
- Identify care gaps, optimize medications, and coordinate individualized care plans with members of the care team and caregivers.
- Use integrated technology and AI-enabled workflows to reduce administrative burden and improve member interaction.
