What you'll do
- Conduct telehealth video visits and occasional in-home visits to local members.
- Perform evidence-based assessments and care focused on prevention, chronic conditions, medication optimization, and health maintenance.
- Identify care gaps and coordinate with care teams to develop individualized care plans.
