Provides preventative, primary, and chronic condition care to older adults through mostly video visits and occasional local in-home visits, coordinating with an interdisciplinary care team.

- Conduct comprehensive assessments and evidence-based preventive care, chronic condition support, medication optimization, and health maintenance.
- Identify care gaps and coordinate individualized care plans with internal and external care team members.
- Use integrated technology and AI-enabled workflows to reduce administrative work and increase member interaction time.
