Provides preventative and primary care to members, primarily through telehealth, including comprehensive assessments, chronic condition support, and coordinated care planning.
What you'll do
- Conduct video visits and occasional local in-home visits; perform evidence-based assessments focused on prevention, chronic conditions, medication optimization, and health maintenance.
- Identify care gaps and coordinate with care teams to develop individualized care plans.
- Use technology and AI-enabled workflows to reduce administrative work and support member interaction.
