About the job
Supports members after hospital discharge by coordinating follow-up care and resources to help them recover safely at home and avoid readmission.
What you'll do
- Contact members after discharge to check on their well-being and needs; coordinate services such as transportation, DME, meals, and home health care.
- Help schedule post-discharge visits and obtain discharge records; collaborate with patients, providers, caregivers, and the TOC team.
- Escalate patient needs to the clinical team and participate in pilot programs and interdisciplinary huddles.
What they ask for
- Active, clear Medical Assistant Certification through a nationally accredited agency (CCMA, AAMA, NHA); proof is required.
- 2–3 years of experience in healthcare, managed care, home health, and/or community health.
Nice to have
- Fully bilingual in English and Spanish.
Pay
$23.00 - $27.00 / hour (base salary)
