About the job
Provides telephonic and virtual clinical triage, advice, and care coordination to Devoted Health members, directing them to appropriate care and resources.
What you'll do
- Provide clinical advice and education by phone and patient portal, and direct members to the appropriate level of care.
- Coordinate care and deliver support through video telehealth visits for urgent complaints.
- Monitor portal chat, respond to messages, escalate active symptoms, and collaborate with the multidisciplinary care team.

- Active, unencumbered Compact RN license or willingness to obtain additional state licenses as needed.
- At least 5 years of direct patient care experience.
- Prior clinical triage experience in an Emergency Department, Intensive Care Unit, Primary Care, or Telephonic Triage setting.
